Are you a dedicated and compassionate nurse with expertise in palliative care? We invite you to consider a rewarding opportunity to join our dynamic Specialist Palliative Care Team (SPCT) in Pembrokeshire as a part-time (30 hours) Band 6 Acute Palliative Care Nurse.
Role Responsibilities
In this role, you will play a vital part in providing exemplary palliative and end-of-life care within an acute environment. Your responsibilities will include:
- Managing a designated caseload of patients under the supervision of clinical nurse specialists and consultants.
- Collaborating with community and hospital staff to ensure comprehensive and accurate care plans are established for all patients.
- Facilitating discussions around advance care planning, ensuring that patients' wishes and preferences are prioritized.
- Providing essential information to patients and their families to empower informed decision-making regarding treatment and care options.
- Ensuring a seamless transition from hospital to home, maintaining continuity of care.
- Developing and delivering educational programs for staff to enhance their skills and knowledge in palliative care.
- Serving as a resource for complex situations, utilizing your communication and negotiation skills to navigate challenging discussions.
- Offering emotional support to families and staff during difficult times.

Qualifications and Skills
We are looking for candidates who possess:
- Level 1 Registered Nurse (Adult) status with current NMC Registration.
- Relevant post-registration qualifications or equivalent experience.

- Experience in acute or community settings, particularly with patients requiring palliative or end-of-life care.
- Strong communication skills, with the ability to engage sensitively with patients and their families.
