About the Role

We are seeking a highly motivated and compassionate Registered General Nurse to join our Specialist Palliative Care Team in Pembrokeshire. As a Part-time Palliative Care Nurse, you will play a vital role in delivering high-quality care to patients with palliative and end-of-life needs.

Main Responsibilities
  • Assess and manage a defined caseload of patients with palliative care needs, working closely with the Specialist Palliative Care Clinical Nurse Specialists and Consultant.
  • Support hospital teams in identifying patients with palliative care needs, ensuring timely and effective care planning.
  • Develop and deliver teaching programs for relevant staff and teams, promoting best practice in palliative care.
  • Communicate effectively with patients, families, and healthcare professionals, providing emotional support and guidance.
  • Collaborate with the community team to ensure seamless care transitions and patient-centered outcomes.

Requirements
  • Level 1 Registered Nurse (Adult) qualification.
  • Current NMC registration.
  • Experience in palliative care, with a strong understanding of advance care planning and end-of-life care.
  • Excellent communication and interpersonal skills, with the ability to work effectively in a team.
  • Ability to work flexibly, including evenings and weekends, as required.
