About the Role

The Hywel Dda University Health Board is seeking a highly motivated and compassionate Registered General Nurse to join their Specialist Palliative Care Team in Pembrokeshire. As a Part Time Palliative Care Nurse, you will be instrumental in delivering first-class palliative and end of life advice, support and care within the acute setting.

The ideal candidate will possess exceptional communication skills and emotional intelligence, being able to skilfully and sensitively communicate with patients, families/care givers and allied health professionals. You will join a team of dedicated Clinical Nurse Specialists, Consultant/Speciality Doctor, Therapists and the wider community MDT in providing expert, high quality patient focused care.

Main Responsibilities

Have responsibility for a defined and delegated caseload of patients under the guidance of the specialist palliative care clinical nurse specialists and consultant.
Support community and hospital staff to ensure that all patients have a clearly documented and accurate plan of care which reflects the assessment undertaken and incorporates the issues and recommendations made, including advance care decisions and do not attempt resuscitation discussions.
Ensure patients are given opportunity to discuss advance care planning openly and that their wishes and preferences are respected and care is co-ordinated.
Ensure patients receive the relevant information to enable them and their relatives to make an informed choice about their treatment and future care.
Facilitate effective and safe transfer from hospital to enable continuity of the care plan beyond transfer of care.

Person Specification

The ideal candidate will have a Level 1 Registered Nurse (Adult) qualification, current NMC Registration, relevant post registration diploma qualification or equivalent experience, evidence of continuing professional development, knowledge of advance care planning and other issues relating to people with palliative and end of life care needs, knowledge of Mental Capacity Act, Safeguarding Adults, Deprivation of Liberty Safeguards, Independent Mental Capacity Advocate policies and guidelines. Desirable qualifications include a teaching qualification, understanding of Human Rights, Mental Capacity Legislation and Continuing Health Care Policy, awareness of clinical governance, knowledge of Continuing Health Care and NHS Funded Nursing Care. The ideal candidate will also have significant post registration experience in an acute or community setting, experience of working with patients with palliative or end of life care needs, experience of complicated assessment and clinical decision making skills. Desirable experience includes supervisory management experience, experience in research and audit programmes, experience of teaching. A Welsh Speaker (Level 1) is desirable for this post.
