Position Overview

This role presents a unique opportunity to engage in primary healthcare while providing exceptional services to the community. The position allows a Primary Care Practitioner to evolve as a healthcare provider.

The Primary Care Practitioner will operate within a healthcare facility, focusing on both urgent and long-term health management. As part of a collaborative healthcare team, the practitioner will participate in urgent care visits, management of elderly patients, chronic illness care, home visits, advanced care planning, and end-of-life care, all while adhering to their professional scope.

Comprehensive supervision and guidance will be provided to the Primary Care Practitioners.

There will be opportunities to engage in quality enhancement initiatives.

We encourage applications from both seasoned and newly qualified practitioners for this position, and we will continue to support them with access to regular educational sessions, clinical development, and mentorship. Peer support and guidance will be available for all practitioners within the network. For those new to primary care, a structured onboarding process will be facilitated.

Key Responsibilities

The following are the primary responsibilities of the Primary Care Practitioner. Additional tasks may be required based on workload and staffing needs. The practitioner will operate within their clinical capabilities and:

• Serve as the initial point of contact for patients presenting with undiagnosed health issues, utilizing comprehensive history-taking, physical assessments, and clinical judgment to formulate a diagnosis and management strategy in collaboration with the patient.

• Conduct thorough examinations, assessments, and diagnoses, providing necessary clinical care and management.

• Analyze and act upon diagnostic test results, collecting or requesting laboratory specimens as needed.

• Assist patients in understanding and utilizing their prescribed medications or over-the-counter options (within their professional scope).

• Deliver integrated, patient-centered care through effective collaboration with the broader healthcare team and social service networks.

• Ensure continuity of care by arranging follow-up consultations or reviews as required.

• Provide routine care to patients in accordance with clinical guidelines and evidence-based practices.

• Utilize clinical protocols, advocate for evidence-based practices, and participate in clinical audits and significant event reviews.

• Adhere to relevant patient group directives and local clinical pathways consistently.

• Support the clinical team with safeguarding matters in line with local and national policies.

• Administer immunizations as necessary.

• Maintain accurate clinical documentation in compliance with current regulations.

• Ensure effective use of clinical coding.

• Work within their professional scope and seek assistance when necessary.

Qualifications

Essential

  • Experience as a Primary Care Practitioner
  • Experience in triage processes
  • Experience managing a variety of clinical conditions
  • Postgraduate diploma or degree in Physician Associate studies
  • Membership in the Physician Associates Managed Voluntary Register
  • Current professional registration in the UK

Desirable

  • Experience in a primary care setting
