About the Role

Salford Primary Care Together (SPCT) is seeking a skilled and dedicated Nurse Practitioner to join our team. As a key member of our Enhanced Access Services, you will play a vital role in delivering high-quality patient care and contributing to the success of our clinical hub.

Key Responsibilities

  • Undertake nursing and therapy tasks, including cytology, chronic disease management, health checks, vaccinations, and lifestyle advice.
  • Manage the care of patients with long-term conditions, assessing, planning, implementing, and evaluating treatment plans to meet identified needs and promote health and well-being.
  • Assess patient needs, providing appropriate care and treatment in conjunction with the GP or independently, referring to colleagues or other agencies as necessary.
  • Make every contact count by offering support and advice on how to improve health and wellbeing, with training and support provided to help signpost patients to other services.
  • Maintain accurate and contemporaneous records to relevant professional standards and guidelines.
  • Support the provision of EPIC Services and promote the fulfillment of the rota.
  • Monitor stock levels and ensure stock is ordered and maintained as required.

Requirements

  • Registered with the Nursing and Midwifery Council.
  • Certified Cytology Sample Taker.
  • Current Level 3 Safeguarding for Adults & Children Certificate.
  • Current Basic Life Support Certificate.
  • Experience of supporting patients with long-term conditions.
  • Experience of undertaking a wide range of other nursing services, including venepuncture, ECG measurement, blood pressure measurement, urinalysis, health checks, lifestyle advice, weight management, smoking cessation, vaccination, and immunisations.
  • Experience of working in a primary care environment.
  • Competent in the use of patient record management systems such as EMIS.
  • Flexible approach to working hours to meet the needs of the service.
