Key Responsibilities

As a member of our dedicated nursing team within the prison, you will collaborate with a multidisciplinary group to provide the following services: Primary Care services, including initial custody assessments, secondary evaluations, nursing clinics, chronic condition management, and health promotion; Inpatient Unit; Emergency Response; and Addiction Recovery nursing services. You will be supported by a welcoming team of healthcare professionals in a dynamic environment where each day brings new experiences.

We are seeking a Band 6 nurse from diverse backgrounds. While experience with patients facing substance misuse and mental health challenges, or in a correctional setting, is advantageous, it is not mandatory as comprehensive training and support will be provided.

Job Duties

PRIMARY OBJECTIVES & KEY TASKS:

To deliver specialized addiction recovery care to offenders and collaborate within an integrated, multidisciplinary health service, which may encompass in-reach services, reception duties, resource center operations, dual diagnosis management, and primary care nursing. To implement a variety of specialized interventions at both primary and secondary care levels, including:
  • Referral management, screening assessments, triage, and evidence-based interventions
  • Individual and group therapy facilitation
  • Assertive outreach interventions
  • Managing a diverse and challenging caseload
To aid in reducing the necessity for inpatient admissions and, when needed, facilitate early discharges from inpatient services through the application of addiction management clinical interventions and complex case management, ensuring continuity of care for offenders. To conduct thorough assessments, screenings, and interventions for offenders with substance misuse issues. Ensure that all waiting times, assessments, and interventions are conducted promptly, contributing positively to the service's performance targets. Foster integrated and collaborative working relationships with other specialized services, ensuring continuity of care and effective transition arrangements. Collaborate closely with community mental health teams (CMHTs) to guarantee appropriate information sharing, continuity of care, and the Care Programme Approach (CPA) for all offenders as required. Provide comprehensive and timely reports and mental health expertise to multi-agency public protection panels (MAPPA), law enforcement, probation services, legal representatives, and court liaison teams as directed. Supervise junior staff and receive supervision as required (both line management and clinical). Participate in resource center services as directed, delivering group therapy and specialized activities under the guidance of the team leader. Engage with offenders in a psychologically informed manner to help them achieve their agreed health goals and quality outcomes. While there is no direct budgetary responsibility, be mindful of the financial implications of decision-making. Maintain close working relationships with all prison and Serco staff, participating in sentence planning, resettlement, safer custody, and ACCT processes. Develop and assume a specialist lead role as appropriate, providing peer support and training in this area. Contribute to the support, advice, consultation, and training for other prison departments. The post holder will be part of skilled multidisciplinary teams comprising all healthcare staff, including strategic partners, managing a single referral pathway. The post holder will be required to undertake training to maximize the uptake of screening, health promotion, and preventive activities, including vaccinations. Ensure that a named care coordinator is assigned to every patient with complex needs, facilitating a proactive, evidence-based approach to clinical interventions, health promotion/prevention activities, and management of acute and chronic conditions using a stepped care model. Ensure that a unified, integrated care plan is developed and that individualized care is regularly reviewed. Conduct comprehensive risk assessments and care plans, ensuring they are regularly reviewed in a needs-led cycle. Tailor care coordination criteria and MDT meetings for patients with complex needs to the specific requirements of the establishment. Undertake training in evidence-based CBT approaches to enhance the consistency and quality of care delivery. Support patients in managing their health at every stage, from oral health to chronic conditions. Ensure that health promotion is embedded into every aspect of the service.

IMPORTANT NOTE:

All applicants must be prepared to undergo National Security Vetting to work in a correctional environment. This will be part of the pre-employment checks conducted by Oxleas and the prison vetting team.
