About the Role

We are seeking a skilled and dedicated Reablement Practitioner to join our team at Shared Services Partnership. As a Reablement Practitioner, you will play a vital role in supporting individuals to live independently in their own homes, promoting their well-being and quality of life.

Key Responsibilities
  • Conduct holistic assessments of need and develop personalized support plans to meet the unique requirements of each individual.
  • Work collaboratively with a multidisciplinary team of healthcare professionals to deliver high-quality care and support services.
  • Provide therapy-based interventions and support to individuals, promoting their independence and autonomy.
  • Develop and maintain strong relationships with individuals, their families, and carers, ensuring that their needs are met and their voices are heard.
  • Stay up-to-date with best practices and industry developments, applying this knowledge to improve the quality of care and support services.
Requirements

To be successful in this role, you will need:

  • A strong understanding of the principles of reablement and the importance of promoting independence and autonomy.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with individuals, families, and carers.
  • Experience of working in a healthcare or social care setting, with a focus on reablement or rehabilitation.
  • A commitment to delivering high-quality care and support services, with a focus on promoting the well-being and quality of life of individuals.
