The ideal candidate will take charge of evaluating and treating adults and elderly patients suffering from various neurological disorders, including Early Supported Discharge (ESD) for stroke patients. Expertise in neurological medications and continence management is highly desirable. Collaboration across disciplines is crucial, particularly in service enhancement and team education. Our team comprises occupational therapists, physiotherapists, speech and language therapists, dieticians, clinical neuropsychologists, neuronavigators, Therapy Assistant Practitioners, administrative staff, and psychology assistants. The successful candidate will play a significant role in the development of community neuro rehabilitation services and lead initiatives within broader neuro nursing pathways. We prioritize ongoing professional growth through clinical supervision, annual Personal Development Plans, and both post-graduate and in-service training from a uni-professional and multi-disciplinary perspective. Mentoring students is strongly encouraged. Our staff receive comprehensive support from the Clinical Team Leader of the Community Neuro Team.Key Responsibilities
Function as an autonomous, advanced practitioner delivering expert nursing services in community neuro rehabilitation. Provide exceptional hands-on nursing care, guidance, support, and education to clients, caregivers, and families with complex neurological needs. Ensure that patients and their families are thoroughly informed and supported through excellent clinical knowledge and assistance. Contribute to the advancement of evidence-based clinical nursing practices aligned with current research and guidelines. Establish clear communication channels within specialty pathways to ensure cohesive multidisciplinary patient management.
