Position Overview
We are pleased to announce an exceptional opportunity for a Pharmaceutical Care Specialist to join our dedicated team at Hanham Health. This role is integral to providing comprehensive pharmacy services across our GP practices in South Gloucestershire. The successful candidate will engage in direct consultations with patients, as well as manage medication inquiries and reviews through various communication channels.
Key Responsibilities
Core Responsibilities
Collaborate with the Hanham Health Prescribing Team and community pharmacists to ensure seamless medication reconciliation for patients following hospital discharge. Deliver pharmaceutical medication assessments and health education to patients at designated practices through in-person consultations, telephone discussions, or documented reviews as per practice agreements. Provide pharmaceutical expertise in long-term condition management clinics as necessary. Organize and conduct medication review sessions, ensuring appropriate follow-up and monitoring of changes. Establish and manage pharmacist-led clinics aimed at evaluating medication efficacy by assessing clinical signs and symptoms, particularly in areas such as minor ailments, anticoagulation, asthma, diabetes, and hypertension. Participate in public health initiatives, including vaccination campaigns for flu, childhood immunizations, and adult immunization programs. Serve as a reliable source of pharmaceutical information for the practice team and patients, addressing inquiries regarding dosages, side effects, adverse reactions, and alternative options. Enhance prescribing practices through educational support for all prescribers within the practice. Lead initiatives where evidence-based changes necessitate updates in prescribing practices across patient demographics, particularly in response to drug withdrawals or changing indications.About Hanham Health

Additional Responsibilities
Support enhancements in clinical care through practice-based audits and implementation of necessary changes. Analyze and present data to identify issues and risks, aiding in informed decision-making. Collect and lead qualitative and quantitative analyses to contribute to various projects. Prepare reports summarizing the status of issues, evaluating outcomes, and providing updates for the Lead Medicines Optimisation Pharmacist and Practice GP Prescribing Lead. Implement drug withdrawal protocols and alerts to improve medication safety. Advocate for the use of electronic prescribing services within the practice. Optimize the use of repeat dispensing services effectively. Ensure adherence to the repeat prescribing policy, conducting medication reviews at appropriate intervals. Manage the repeat prescribing reauthorization process by reviewing requests for repeat prescriptions and monitoring medications due for review. Provide a helpline for patients with inquiries regarding their medications. Coordinate with community pharmacies regarding transitions from care settings and the utilization of monitored dosage systems.Candidate Profile
Experience Requirements
Minimum of one year of post-registration experience. Previous experience working within a multidisciplinary team.Qualifications
Pharmacy Degree. Registration with the General Pharmaceutical Council. Demonstrated commitment to continuous professional development.Skills and Attributes
