As a Targeted Support Coordinator, you will play a crucial role in assisting our clients with complex needs, empowering them to enhance their skills and achieve greater independence within their communities.
Key Responsibilities:
- Provide comprehensive support and guidance to vulnerable clients who have faced challenges such as rough sleeping.
- Establish and maintain strong professional relationships with clients to encourage engagement and minimize tenancy breaches.
- Manage a caseload of up to ten clients, adapting support plans to meet individual needs.
- Conduct thorough needs assessments and risk evaluations to develop effective support strategies.
- Facilitate access to essential services, including healthcare and social support, to help clients maintain their tenancies.
- Encourage the development of social networks and self-sufficiency among clients.
- Assist clients with daily living tasks, including budgeting, meal preparation, and shopping.
- Collaborate with colleagues and external organizations to ensure a coordinated approach to client care.
- Document progress and outcomes through quarterly case studies and reports.
Your Qualifications:
- A genuine passion for supporting individuals in achieving their best lives.
- Experience in creating personalized support plans and collaborating with team members.
- Proven ability to assess client referrals and coordinate appropriate services.
- Strong problem-solving skills and the ability to work independently under pressure.
- Previous experience working with vulnerable populations or relevant lived experience.
- Understanding of social housing and welfare benefits.
- Excellent communication and negotiation skills, with a track record of successful partnership working.
- A valid driving license and access to a vehicle for work purposes.
