- Provide ongoing support, intervention, and monitoring to ensure that care plans are effective and updated as needed.
- Build and maintain strong, trusting relationships with service users, families, and carers.
- Ensure compliance with relevant legislation, policies, and procedures related to child protection and family support.
- Qualified Social Worker registered with Social Work England.
- Proven experience in a social work role, particularly in long-term or ongoing case management.
- Knowledge of relevant legislation, policies, and procedures related to social care and child protection.
