About the Role
We are seeking a dedicated and experienced Mental Health Community Nurse to join our team at Hertfordshire Partnership University NHS Foundation Trust (HPFT). In this role, you will play a vital part in providing high-quality care to individuals with severe mental illness and learning disabilities within their homes and communities.
Key Responsibilities
- Deliver compassionate and effective nursing care to patients with diverse needs, ensuring their physical and mental well-being is prioritized.
- Collaborate closely with multidisciplinary teams, including doctors, social workers, and therapists, to develop and implement individualized care plans.
- Proactively assess and manage patient risks, implementing appropriate safety measures and interventions.
- Promote the inclusion of individuals with severe mental illness and learning disabilities in mainstream services, advocating for their rights and needs.
- Contribute to service improvement initiatives by participating in audits, data collection, and planning processes.
Essential Requirements
- Registered Nurse with current NMC registration (Mental Health/Learning Disability/General).
- ENB 998 / Preceptorship and or Practice Supervisor and Assessor Training.
- Minimum of 3 years post-registration experience at Band 5 level, with a focus on community nursing.
- Proven experience in managing stressful situations, providing support to both staff and service users.
- In-depth understanding of evidence-based practices related to physical health in individuals with severe mental illness (e.g., The Lester Tool, DoH Improving the Physical Health of People with Mental Health Problems).
- Valid driving license (unless you have a disability as defined by the Equality Act 2010 which prevents you from driving).
Desirable Requirements
- Experience working in clinic settings, with Clozapine, Depot/LAI administration, or physical health.
