Senior Integrated Discharge Practitioner

This is an exciting opportunity to join the Manchester Care Group as a Senior Integrated Discharge Practitioner. As part of the inpatient and wider Manchester leadership team, you will work collaboratively with colleagues to ensure the safe and rapid discharge of service users who no longer require a hospital setting.
Main Duties- Ensure discharge planning from point of admission and identify potential barriers early on.
- Support teams with a positive risk-based approach to discharge, reducing harm associated with long lengths of stay.
- Utilize extensive experience of statutory assessments to ensure they are of the highest standard.
- Act as a 'Trusted Assessor' to complete and review Care Act assessments, reducing delays.
- Work with complex and lengthy delays, progressing cases forward using a multi-partner approach to find long-term, sustainable community-based solutions.
- Collaborate with clinical and operational partners across all services to facilitate discharge and maintain flow across the GMMH footprint.
- Provide operational resilience support, working closely with care groups to develop, implement, manage, and review operational plans, policies, and escalation protocols.
- Case manage the most complex discharge cases, supporting patient flow and working with key partners to create seamless pathways of care from hospital to community and home.

Job Responsibilities- Provide strong and effective clinical leadership and line management for Manchester teams, promoting a positive and therapeutic culture that empowers staff to achieve optimal performance.
- Utilize your own judgment to provide options and resolutions, involving complex facts and professional advice from the MDT, family, and carers to drive a way forward.
- Oversee the daily delivery of discharges, including complex cases and patients clinically ready for discharge.
- Provide effective leadership for the Manchester Patient Flow Service spoke practitioners, promoting a positive and therapeutic culture.
- Support the Care group at internal and external patient flow forums, sharing information, supporting improvement plans, and taking issues forward.
- Participate in local and regional networks to promote Manchester Care Group and patient flow.
- Retain daily oversight of discharge progress, providing live and accurate updates to the Head of Operations, Associate Directors, and Strategic Lead as required.
- Pay enhancements: 30% additional for evenings (8pm onwards) and Saturdays, and 60% additional for Sundays and Bank holidays.
