Job Summary

This is an exciting opportunity to join the Manchester Care Group as a Senior Integrated Discharge Practitioner. As part of the inpatient and wider Manchester leadership team, you will work collaboratively with colleagues to ensure the safe and rapid discharge of service users who no longer require a hospital setting.

Main Duties
  • Ensure discharge planning from point of admission and identify potential barriers early on.
  • Support teams with a positive risk-based approach to discharge, reducing harm associated with long lengths of stay.
  • Utilize extensive experience of statutory assessments to ensure they are of the highest standard.
  • Act as a 'Trusted Assessor' to complete Care Act assessments and reduce delays.
  • Work with complex and lengthy delays, progressing cases forward and finding long-term, sustainable community-based solutions.
  • Collaborate with clinical and operational partners to facilitate discharge and maintain flow across the GMMH footprint.
  • Provide operational resilience support, working closely with care groups to develop and implement operational plans, policies, and escalation protocols.
  • Case manage complex discharge cases and support patient flow, utilizing expert knowledge of community services to create seamless pathways of care.

Job Responsibilities
  • Provide strong and effective clinical leadership and line management for Manchester teams, promoting a positive and therapeutic culture.
  • Utilize professional advice from the MDT, family, and carers to drive a way forward for complex cases.
  • Oversee the daily delivery of discharges, including complex cases and patients clinically ready for discharge.
  • Provide effective leadership for the Manchester Patient Flow Service spoke practitioners, promoting a positive and therapeutic culture.
  • Support the Care group at internal and external patient flow forums, sharing information and taking issues forward.
  • Participate in local and regional networks to promote Manchester Care Group and patient flow.
