About the Role
As a Dementia Advisor, you will play a vital role in providing expert guidance and support to individuals living with dementia, their families, and healthcare professionals within our community.
You will be a dedicated point of contact for clients following diagnosis at the memory clinic, ensuring they receive holistic care tailored to their needs. Your expertise will contribute to developing and implementing effective strategies that enhance the well-being of those affected by dementia.
Key Responsibilities- Provide specialized advice and support to colleagues, clients, and carers regarding dementia care.
- Collaborate effectively with multidisciplinary teams and agencies to deliver comprehensive care plans.
- Conduct pre-screen assessments as directed by the Clinical Nurse Specialist (CNS).
- Supervise and mentor junior staff members, sharing your knowledge and experience in dementia management.
- Contribute to the development and implementation of policies and procedures related to dementia care.
You will be a Registered Mental Health Nurse with a strong commitment to providing high-quality dementia care. Your experience in managing patients with dementia and associated conditions, coupled with your ability to work collaboratively within multidisciplinary teams, will be essential for success in this role.
A passion for continuous learning and improvement, along with excellent communication and interpersonal skills, are highly valued.
