About the Role

We are seeking a highly skilled and experienced Clinical Triage Practitioner to join our team at Queen Elizabeth's Foundation for Disabled People. As a key member of our rehabilitation service, you will be responsible for assessing clients with complex neurological conditions and making recommendations for their care and rehabilitation.

Key Responsibilities
  • Undertake specialist holistic rehab assessments, including those with complex presentations and multi-pathologies, using clinical reasoning skills.
  • Control and manage triage calls effectively, using skilled questioning, active listening, and excellent verbal communication skills.
  • Evaluate patient progress, write reports, and assessments, and be responsible for the handover process to relevant QEF CRC clinical staff.
  • Communicate effectively with patients, family members, carers, and colleagues, using tact and persuasive skills.
  • Work in line with QEF policies and procedures, and within your own professional body, demonstrating a good working knowledge of national and local standards of practice.
Requirements
  • 5+ years post-qualification clinical experience, including 3+ years in a community setting and 3+ years working with neurologically impaired adults.
  • Solid experience of working in a team/MDT environment, with experience of working autonomously and carrying a caseload.
  • Ability to demonstrate and evidence effective clinical skills, equivalent to Band 6 competency level.
  • Current NMC or HCPC registration, and completed mentorship and/or clinical supervision training.
