Clinical Triage Practitioner

We are seeking a highly skilled and experienced Clinical Triage Practitioner to join our team at Queen Elizabeth's Foundation for Disabled People. As a key member of our rehabilitation service, you will be responsible for assessing and triaging clients with complex neurological conditions, making recommendations for their care and rehabilitation, and working closely with our multidisciplinary team to ensure seamless transitions of care.
Key Responsibilities- Conduct thorough assessments of clients with complex neurological conditions, using your expertise and clinical reasoning skills to identify their needs and develop personalized rehabilitation plans.
- Make recommendations for client care and rehabilitation, taking into account their individual needs and goals.
- Work closely with our multidisciplinary team, including therapists, nurses, and other healthcare professionals, to ensure seamless transitions of care and effective communication.
- Develop and maintain strong relationships with clients, their families, and carers, providing emotional support and guidance throughout their rehabilitation journey.
- Stay up-to-date with the latest research, best practices, and clinical guidelines in neurological rehabilitation, applying this knowledge to inform your practice and improve client outcomes.
- Registered nurse or postgraduate physiotherapist/occupational therapist with current NMC or HCPC registration.
- Minimum 5 years of post-qualification clinical experience, with a strong background in neurological rehabilitation.
- Excellent communication and interpersonal skills, with the ability to work effectively with clients, families, and healthcare professionals.
- Strong clinical reasoning and decision-making skills, with the ability to assess complex client needs and develop effective rehabilitation plans.
- Ability to work autonomously and as part of a multidisciplinary team, with a strong commitment to teamwork and collaboration.
