Community Mental Health Nurse Specialist

We are seeking a skilled and motivated Mental Health Nurse to join our Community Mental Health Team. As a key member of the team, you will be responsible for providing high-quality care to individuals with severe and enduring mental health problems.
Key Responsibilities- Provide care coordination and case management for a defined caseload of clients with complex and challenging needs.
- Ensure that care and treatment plans are adhered to and that standards are met in line with the Mental Health Measure (Wales) 2010.
- Formulate and develop standard and complex outcome-focused care and treatment plans.
- Conduct comprehensive risk assessments and document findings.
- Support clinical duties such as assisting with the running of the depot/clozapine clinic and the administration and supervision of long-acting injections.
- Complete physical health assessments and side-effect monitoring for patients prescribed Clozapine.
- Assess and evaluate individuals in crisis, completing comprehensive assessments of their health and social care needs and agreeing the most appropriate course of action.
- Formulate plans of care and interventions based on identified need and deliver evidence-based crisis interventions as appropriate.
- Work collaboratively with service users, carers, and partner agencies in delivering high-quality care.
