Job Overview

The Betsi Cadwaladr University Health Board is seeking a skilled and motivated Mental Health Nurse to join our established integrated, multi-disciplinary team. As a key member of the Community Mental Health Team, you will be responsible for the care coordination and case management of a defined caseload of clients with complex and challenging needs.

Main Duties

• To be responsible for the care coordination of a designated caseload of clients with severe and enduring mental health problems.

• To ensure that the Care and Treatment Plans are adhered to and that standards are met in line with the Mental Health Measure (Wales).

• To act as a point of contact for GPs and other interface services to offer advice, support, and signposting to appropriate agencies.

• To formulate and develop standard and complex outcome-focused care and treatment plans.

• To ensure that a comprehensive risk assessment is completed and documented for actual and potential risk.
