Job Title: Community Mental Health Nurse

We are seeking a skilled and motivated Mental Health Nurse to join our Community Mental Health Team in Nant y Glyn Secondary Care. The successful candidate will be responsible for the care coordination and case management of a defined caseload of clients with complex and challenging needs.

Main Responsibilities:
  1. Develop and implement comprehensive care plans in collaboration with clients, carers, and other healthcare professionals.
  2. Conduct thorough assessments of clients' mental health needs and develop strategies to address these needs.
  3. Provide high-quality care and support to clients, including medication management and crisis intervention.
  4. Work collaboratively with other healthcare professionals to ensure seamless care coordination and effective communication.
  5. Participate in multidisciplinary team meetings to discuss client care and develop strategies to improve outcomes.

Requirements:
  • Registered Mental Health Nurse with current registration with the NMC.
