Job Responsibilities- Provide holistic assessments and care planning for patients with complex needs
- Work collaboratively with the multidisciplinary team to prevent hospital admissions and accelerate discharge
- Develop and implement individualized care plans to promote independence and well-being
- Conduct regular reviews and updates to ensure patient needs are met
- Registered Nurse (Adult) with NMC registration
- Relevant post-registration study at degree level and relevant Continuing Professional Development (CPD)
- Mentorship course or equivalent
- Experience of conflict management and managing complex patient needs
