Job Overview:
The Avon and Wiltshire Mental Health Partnership NHS Trust is seeking a highly skilled Consultant to provide medical expertise to support the Memory Service and Care Home Liaison team. The successful candidate will work within three dedicated teams focused on specific areas within the Clinical Commissioning Group region.
Key Responsibilities:
- Deliver direct interventions to service users with complex mental and physical health needs, including those who are difficult to engage.
- Regular attendance at Multi-Disciplinary Team meetings to provide expert advice and guidance.
- Conduct assessments and treatment of outpatients, ensuring comprehensive care and support.
- Hold CPA meetings for outpatients, providing a safe and supportive environment for discussion and planning.
- Visit patients in their place of residence, offering personalized care and attention.
- Liaise with other service areas and medical professionals to ensure seamless care coordination.
- Prepare and present Mental Health Act Tribunal reports, demonstrating expertise and professionalism.
- Participate in Mental Health Act Tribunal meetings, advocating for the rights and needs of service users.
- Provide medical and AC responsibility for designated GP practices, ensuring high-quality care and support.
