Position Overview

The inpatient pharmacy at Somerset NHS Foundation Trust is dedicated to ensuring the safe and effective distribution of medications to its patients, adhering to all applicable regulations.

Our medicines optimisation team is committed to delivering a cohesive and patient-focused service throughout the trust, promoting equitable access, safety, and cost-effectiveness while adhering to evidence-based practices.

The clinical pharmacy team plays a vital role in providing expert guidance and support to enhance medication management and improve patient outcomes, working collaboratively as part of multidisciplinary teams within the healthcare system.

Key Responsibilities

Dispensary Duties

- Provide in-patient medication supplies and discharge prescriptions to hospital wards.

- Manage the dispensing of medications for various commercial contracts.

- Supply medications to affiliated care facilities.

- Handle clozapine prescriptions and navigate associated dispensing platforms.

Out-Patient Services

- Oversee the management of out-of-stock medications, coordinating with procurement to ensure a reliable supply.

- Work towards achieving accreditation for final accuracy checking (FACP).

Ward Services

- Engage in the regional Medicines Optimisation initiative and obtain relevant accreditations.

- Once accredited, collaborate with technicians and pharmacists on the wards to deliver comprehensive medication management services, which include:

  • Verification of Patients Own Drugs (PODs)
  • Discharge dispensing through a streamlined process
  • In-patient medication supply
  • Patient education and counselling
  • Facilitation of discharge processes
  • Medication reconciliation
  • Reporting of interventions

Weekend and holiday shifts will be required on a rotational basis.

Aseptic Services

- Participate in the routine preparation of aseptically manufactured products within the pharmacy unit.

- Ensure a clean and safe environment for the preparation of aseptic products.

- Engage in the validation of manufacturing techniques and related processes.

- Work towards qualifying as a 'Pre and In-Process' checking technician, which includes verifying ingredients, worksheets, labels, drug volumes, and product integrity.

Education and Training Responsibilities

- Contribute to the training of technicians, support staff, students, and ward personnel.

- Participate in the onboarding of new staff members.

- Maintain personal registration and ensure that accreditation certifications are achieved and kept up to date.

Qualifications

Essential

  • B Tech/City and Guilds/Apothecary Hall or NVQ3 with B Tech in Pharmaceutical Sciences or equivalent.
  • Registration with the General Pharmaceutical Council (GPhC) as a Pharmacy Technician.

Desirable

  • Accreditations in medication management and optimisation.
  • Accreditation in Accuracy Checking.

Experience

Essential

  • Prior experience in a dispensary setting, including inpatient, outpatient, commercial, and private prescriptions.

Desirable

  • Experience in training others.

Additional Criteria

Essential

  • Proficiency in computer applications, including Microsoft Word and email.
