Job Title: Adults Team Leader Adult Community Services

We are seeking a highly skilled and experienced Adults Team Leader to join our team at the Transformation Unit. As a key member of our community rehabilitation team, you will play a vital role in leading and managing our specialist services, ensuring the highest quality patient care and outcomes.

Main Responsibilities:
  • Lead and manage the specialist Community Rehabilitation team, Discharge to Assess Team, and Intermediate Home Care Pathway Team.
  • Provide operational leadership and support to team members, ensuring they have the necessary skills and resources to deliver high-quality patient care.
  • Develop and implement clinical service development plans, working closely with colleagues across central adult and specialist community services.
  • Collaborate with multidisciplinary teams to ensure seamless patient care and outcomes.
  • Contribute to the development and implementation of policies and procedures, ensuring compliance with regulatory requirements.
Person Specification:
  • Recognised BSC Honours degree in occupational therapy or physiotherapy.
  • State registration via Health Care Professionals Council.
  • Completion of validated/nationally recognised postgraduate specialist/advanced level courses or equivalent.
  • Evidence of relevant postgraduate training, including attendance at relevant specialist courses/conferences.
  • MSc in a relevant field.
  • Completion of a postgraduate management course.
Knowledge and Experience:
  • Extensive clinical experience, with experience of working at senior level in a relevant specialist field and team management.
  • Advanced and comprehensive knowledge of physiotherapy assessment and treatment techniques.
  • Evidence of recent relevant Continual Professional Development to an advanced level.
  • Experience of a commitment to inter- and multidisciplinary team working.
  • Experience of supervision of qualified staff, students, and assistants.
  • Experience in a post at this level.
  • Evidence of involvement in governance activities.
