Job Summary

We are seeking a highly skilled and experienced Therapy Operational AHP Lead to join our multidisciplinary team at the community hub in Gorton. This dynamic new role will play a crucial part in improving patient pathways on discharge from hospital and ensuring the best possible patient experience of rehabilitation.

The successful candidate will provide exceptional operational leadership to our specialist community rehab team and Manchester Response teams, including Discharge to Assess. They will work collaboratively with colleagues to ensure a seamless patient journey, providing education and support as needed.

The role will involve leading on the planning, delivery, and evaluation of CPD, clinical audit, research, and training. The successful candidate will also be responsible for managing and providing leadership for the specialist Community Rehabilitation team, the Discharge to Assess Team, and the Intermediate Home Care Pathway Team.

Main Duties
  • Provide operational leadership to the specialist Community Rehabilitation team, the Discharge to Assess Team, and the Intermediate Home Care Pathway Team.
  • Manage and lead on the planning, delivery, and evaluation of CPD, clinical audit, research, and training.
  • Work collaboratively with colleagues to ensure a seamless patient journey.
  • Lead clinical service development within the specialist field, working closely with colleagues across central adult and specialist community services.

Requirements
  • Recognised BSC Honours degree in occupational therapy or physiotherapy.
  • State registration via Health Care Professionals Council.
  • Completion of validated/nationally recognised postgraduate specialist/advanced level courses or equivalent.
  • Evidence of relevant postgraduate training, including attendance at relevant specialist courses/conferences.
Clinical Skills and Abilities
  • Excellent interpersonal skills and communication skills.
  • Good organisational, planning, prioritising skills.
  • Ability to motivate and negotiate to encourage reluctant and challenging patients.
  • Highly developed concentration skills, and advanced analytical, problem-solving, and clinical reasoning skills.
