About the Role

We are seeking a highly skilled and experienced Mental Health Liaison Practitioner to join our team at Pennine Care NHS FT. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality mental health services to patients in the acute trust.

Key Responsibilities
  • Assess patients' mental health needs and facilitate access to appropriate services.
  • Manage risks and liaise with family and agencies as needed.
  • Ensure safe discharge of patients back to their GP.
  • Participate in prioritizing referrals and managing workload.
  • Provide evidence-based assessments and short-term interventions for individuals requiring mental health services.
  • Maintain accurate records and input data into information systems.
  • Contribute to service development and participate in team business and practice meetings.
  • Support learning outcomes for student placements within the team.
  • Adhere to operational policies and participate in audits to monitor quality of care.
Requirements
  • Professional registration in health or social care with up to-date accreditation/CPD.
  • Substantial post-registration experience working in acute mental health services.
  • Experience in working with children/young people.
  • Excellent assessment, care-management, and planning skills.
  • Ability to work effectively under pressure and manage a varied workload.
  • Strong communication and interpersonal skills.
  • Ability to supervise staff, set standards, and promote staff development.
