About the Role

We are seeking a highly skilled and experienced Mental Health Liaison Practitioner to join our team at Pennine Care NHS FT. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality mental health services to patients in the acute trust.

Key Responsibilities

  • Assess patients' mental health needs and provide effective interventions to support their recovery.
  • Work closely with the multi-disciplinary team to ensure seamless care coordination and effective risk management.
  • Develop and maintain strong relationships with police, ambulance staff, and other agencies to ensure effective liaison and support.
  • Contribute to the development of the team's gateway and assessment role, ensuring effective interfaces between teams and services.
  • Provide clinical support and guidance to non-mental health colleagues within the hospital, promoting a holistic and person-centered approach to care.

Person Specification

  • Professional registration in health or social care with up to-date accreditation/CPD.
  • Substantial post-registration experience working in acute mental health services, with a focus on crisis liaison and assessment.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
  • Ability to manage complex cases and prioritize tasks effectively, with a focus on delivering high-quality care.
