About the Role

We are seeking an experienced Care Co-ordinator to join our team at Active Care Group. As a key member of our Care in the Home division, you will play a vital role in ensuring the delivery of high-quality care services to our clients.

Key Responsibilities
  • Coordinate and schedule care packages to ensure continuity of care for our clients.
  • Provide exceptional customer service, responding to client and worker inquiries, and resolving any issues that may arise.
  • Collaborate with the team leader to ensure compliance with company policies and procedures, and regulatory requirements.
  • Develop and maintain positive relationships with internal and external stakeholders, including clients, workers, and healthcare professionals.
  • Identify and implement innovative solutions to improve care delivery and client outcomes.
Requirements
  • Experience as a Care Co-ordinator or in a similar role.
  • Proven ability to work effectively in a team environment, with strong communication and interpersonal skills.
  • Ability to manage multiple priorities, work under pressure, and meet deadlines.
  • Strong problem-solving skills, with the ability to think critically and creatively.
  • Excellent customer service skills, with a focus on delivering high-quality care services.
