About the Role

We are seeking a highly skilled and compassionate Clinical Nurse Specialist to join our team at St Peter & St James Hospice. As a key member of our specialist palliative care team, you will provide expert nursing care to patients with complex symptom management needs in the community.

Key Responsibilities:

  • Assess referrals and provide nursing interventions to patients with palliative care needs
  • Work autonomously to manage a caseload of patients, prioritizing their needs and coordinating care with medical and other colleagues
  • Provide palliative nursing interventions, promoting skilled, professional, and compassionate care in the community
  • Identify nursing best practice developments and implement them to support the clinical team
  • Collaborate with internal and external stakeholders to ensure a coordinated and effective service

Requirements:

  • Registered nurse with experience in community nursing and palliative care
  • Excellent communication and assessment skills
  • Ability to work autonomously and prioritize patient needs
  • Knowledge of palliative care best practices and ability to implement them
  • Own transport essential
