Job Summary
We are seeking a highly skilled and experienced Specialist Nurse Practitioner to join our Asthma Clinical Nurse Specialist Team on a 12-month fixed-term contract. As a key member of our team, you will be responsible for managing a defined caseload of patients with complex or chronic conditions, providing specialist advice, and planning and implementing care.
Key Responsibilities
- Assess patients with complex or chronic conditions, providing specialist advice and planning and implementing care.
- Manage own defined caseload of patients, taking responsibility for continuity of care and acting as an independent practitioner.
- Run nurse-led clinics and comprehensive follow-up services, ensuring seamless lines of communication with patients and between secondary and primary care.
- Oversee management of resources, ensuring appropriate use of resources to promote effective patient care and achieve national standards.
Requirements
- Experience in respiratory medicine or a related field.
- Strong clinical skills and ability to work independently.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a team environment.
