Job Summary

We are seeking a highly motivated and enthusiastic Band 6 Nurse Specialist to join our Community Nurses Heart Failure team. The successful candidate will be responsible for assessing and managing pathways for patients and their families with definite and possible Heart Failure conditions.

Main Duties
  • Assess and manage pathways for patients and their families with Heart Failure conditions
  • Develop and implement care plans to improve patient outcomes
  • Provide clinical advice and support to patients, relatives, and healthcare professionals
  • Deliver training and education to staff and patients
  • Collect and monitor National Heart Failure Audit tool data to ensure Trust compliance with local CQUIN targets
About the Role

This is an exciting opportunity to work in a dynamic and supportive team, providing high-quality care to patients with Heart Failure. The successful candidate will be responsible for managing nurse-led clinics and their own caseload, working autonomously and making independent decisions.

Requirements
  • RGN Level one
  • NMC registered
  • Assessing qualification and evidence of teaching ability
  • Continuous professional development
  • First degree as a minimum or working towards
Desirable Qualifications and Experience
  • Non-Medical Prescribing (or willing to undertake)
  • Advanced Practice and Consultation Skills (or willing to undertake)
  • Experience as a Band 6 Community Heart Failure Specialist Nurse
Skills and Knowledge
  • ECG Interpretation or working towards
  • Ability to communicate effectively with individuals and groups
