Community Care Coordinator

We are seeking a highly skilled and dedicated Neonatal Nurse Specialist to join our team at Barking, Havering and Redbridge University Hospitals NHS Trust. As a key member of our Neonatal Community team, you will play a vital role in providing high-quality care and promoting the well-being of our patients and their families.
Main ResponsibilitiesThe successful candidate will be responsible for:
- Providing specialist care and support to preterm and sick infants from hospital to community services
- Assessing, planning, developing, and implementing care plans in conjunction with family/carers
- Working collaboratively with multidisciplinary teams to ensure seamless care transitions
- Promoting continuity of care and improving patient outcomes

To be successful in this role, you will need:
- RN with current registration
- Neonatal qualification
- Current Driving Licence
Desirable qualifications include counselling skills and leadership/management courses.
