About the Role

The Care Coordinator will play a pivotal role in ensuring all patients receive the best possible care and service. This involves working closely with the practices and the multidisciplinary team (MDT) within the Primary Care Network (PCN). The Care Coordinator will support the clinical director and member practices in coordinating all key activities, including access to services, advice, and information, and ensuring health and care planning is timely, efficient, and patient-centered.

Main Responsibilities

The Care Coordinator will be responsible for:

  • Supporting patients to utilise decision aids in preparation for a shared decision-making conversation;
  • Holistically bringing together all of a person's identified care and support needs, and exploring options to meet these within a single personalised care and support plan (PCSP);
  • Helping people to manage their needs through answering queries, making and managing appointments, and ensuring that people have good quality written or verbal information to help them make choices about their care;
  • Supporting people to take up training and employment, and to access appropriate benefits where eligible;
  • Supporting people to understand their level of knowledge, skills, and confidence (their Activation level) when engaging with their health and wellbeing, including through the use of the Patient Activation Measure;

About Us

Healthbridge Direct is a GP federation that aims to strengthen, develop, and support local general practice. We are working together with five Primary Care Networks (PCNs) in Redbridge to build strong clinical teams within and across the PCNs. We are now at the start of an exciting journey of building a team of over 100 additional clinical roles to support and enable the delivery of services within each of our PCNs.

Requirements

The ideal candidate will have:

  • Experience of working within a social care and/or health setting, working with multi-disciplinary teams;
  • Demonstrable experience of effective planning and organisational skills to deliver targets to deadlines;
  • Experience of working with people with diverse health and social care needs;
  • Patient clinical system knowledge;
  • Experience of using recognised assessment tools;
  • Experience of mental health either as a service user, carer, worker, or volunteer;
  • Experience of multi-agency working and signposting to appropriate support;
