Care Coordinator

About the Role
The Care Coordinator will play a pivotal role in ensuring patients receive the best possible care and service. Working closely with the practices and multidisciplinary team within the Primary Care Network, the Care Coordinator will support the clinical director and member practices in coordinating key activities, including access to services, advice, and information. This role will also involve supporting digital initiatives and coordinating patients' journeys through primary care.
Main Responsibilities
- Support patients in utilizing decision aids for shared decision-making conversations
- Develop personalized care and support plans (PCSPs) that meet patients' identified needs
- Help patients manage their needs through answering queries, making and managing appointments, and providing written or verbal information to inform their care choices
- Support patients in accessing training and employment opportunities, as well as benefits where eligible
- Assess patients' activation levels and provide support to enhance their engagement with their health and wellbeing
About Us
Healthbridge Direct is a GP federation that strengthens, develops, and supports local general practice to deliver high-quality care to local people. We are building a team of over 100 additional clinical roles to support and enable the delivery of services within our Primary Care Networks. As a Care Coordinator, you will be part of a dynamic team that works collaboratively to achieve our mission.
Requirements
- GCSE or equivalent NVQ 3 or equivalent qualification
- Experience working in a social care and/or health setting, with multi-disciplinary teams
- Effective planning and organizational skills to deliver targets to deadlines
- Patient clinical system knowledge and experience using recognized assessment tools
- Mental health experience, either as a service user, carer, worker, or volunteer
- Multi-agency working and signposting to appropriate support
