Key Responsibilities- Process repeat medication requests, including the conversion of acute requests to repeat medicines, changing quantities of medicines
- Establish positive working relationships and effective communication with the CCG, network practices, integrated care health and care teams, and a range of local voluntary sector and community organisations
- Document interventions made to patients' medications in their health care records to ensure there is a clear audit trail and changes are clearly accounted for
- GPhC Registration
- NVQ L3 or equivalent qualification in pharmacy services
- Effective communication
- Understanding of pharmacy laws, ethics and terminology
- Knowledge of principle of optimisation
- Good IT skills and knowledge of clinical systems

- Computer literate with an ability to use the required GP clinical systems (EMIS and Docman)
- Understand the aims of current healthcare policy within the PCN
- Right to work in the UK
