Job Title: Band 5 Community Nurse

We are seeking a highly skilled and dedicated Band 5 Community Nurse to join our Haltemprice Community Nursing and Conditions Team. As a Community Nurse, you will play a vital role in providing high-quality care to patients in their own homes, working closely with other healthcare professionals to deliver holistic and patient-centered care.

Key Responsibilities:

  • Provide comprehensive nursing care to patients with long-term conditions, complex wounds, and palliative care needs.
  • Work collaboratively with multidisciplinary teams to develop and implement individualized care plans.
  • Conduct thorough assessments and evaluations to identify patient needs and develop effective care strategies.
  • Deliver education and support to patients, carers, and other healthcare professionals to promote health and well-being.
  • Participate in quality improvement initiatives to enhance patient outcomes and service delivery.

Requirements:

  • Degree in Nursing Studies or equivalent qualification.
  • Registration with the Nursing and Midwifery Council (NMC).
  • Experience of working in a community setting, preferably with patients with long-term conditions.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Basic IT skills and proficiency in using computerized recording systems.
