About the Role:
The Field Care Supervisor is a key position within our organization, responsible for ensuring the delivery of high-quality care support services to our clients. This role requires a strong understanding of care principles and practices, as well as excellent communication and leadership skills.
Key Responsibilities:
- Support the planning and delivery of care services to meet the needs of individual clients
- Conduct regular reviews of client services to ensure they are meeting their needs and are of high quality
- Provide supervision and guidance to care workers to ensure they are competent and confident in their roles
- Work collaboratively with care coordinators to plan and deliver care services
- Participate in multi-disciplinary reviews of client needs and develop care plans to meet those needs
Requirements:
- Experience in carrying out initial risk and needs assessments for individual clients
- Ability to work in partnership with clients, their families, carers, and healthcare professionals to deliver holistic care packages
- Strong leadership and communication skills
- Ability to work independently and as part of a team
- Knowledge of care principles and practices
