About the Role

We are seeking a skilled and compassionate Community Nurse to join our team at Hertfordshire Community NHS Trust. As a Community Nurse, you will be responsible for providing high-quality nursing care to patients in their own homes, promoting health and wellbeing, and working closely with other healthcare professionals to deliver effective care.

Key Responsibilities
  • Assess, plan, implement, and evaluate individualised nursing care to patients living in the community
  • Develop and maintain effective relationships with patients, families, and carers
  • Work collaboratively with other healthcare professionals to deliver comprehensive care
  • Contribute to the development and implementation of care plans
  • Participate in clinical audits and quality improvement initiatives
Requirements
  • Registered General Nurse with a valid NMC registration
  • Experience of working in a community setting
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Flexibility to work a varied shift pattern
