Main Responsibilities:
  • Liaise with multidisciplinary teams, including Social Workers, District Nurses, and other agency professionals, to ensure seamless care delivery.
  • Communicate effectively with customers and their families to understand their needs and preferences.
  • Work collaboratively with colleagues to ensure that customers receive high-quality care and support.
  • Assist customers with their personal needs, such as dressing, washing, meal preparation, and access to their community, as detailed in the Care Plan.
  • Perform light housekeeping tasks, such as dusting, sweeping/vacuuming carpets, and polishing furniture, to maintain a clean and safe environment.
  • Encourage customers to undertake personal care and tasks that they are capable of, recognizing that their abilities may change on a day-to-day basis.
