About the Role

We are seeking an experienced and skilled Psychiatric Liaison Nurse to join our team at the Central and North West London NHS Foundation Trust. As a Psychiatric Liaison Nurse, you will play a vital role in providing high-quality care to patients with mental health needs.

Key Responsibilities
  • Provide comprehensive assessments and care planning for patients with mental health needs
  • Work closely with multidisciplinary teams to ensure seamless care and effective communication
  • Develop and implement individualized care plans that address the unique needs of each patient
  • Collaborate with other healthcare professionals to ensure timely and effective interventions
  • Conduct risk assessments and develop strategies to mitigate risks
  • Provide education and support to patients and their families to promote recovery and well-being
Requirements
  • Registered Mental Health Nurse (RMN) with a valid NMC registration
  • Post-registration experience in a mental health setting
  • Knowledge of mental health legislation, policies, and procedures
  • Ability to communicate effectively with patients, families, and healthcare professionals
  • Strong assessment and care planning skills
  • Ability to work in a fast-paced environment and prioritize tasks effectively
