About the Role

We are seeking a highly motivated and experienced Psychiatric Liaison Nurse to join our team at Central and North West London NHS Foundation Trust. As a Psychiatric Liaison Nurse, you will work closely with individuals, families, and carers to provide high-quality care and support. Your role will involve assessing patients' mental health needs, developing and implementing care plans, and collaborating with healthcare professionals to ensure seamless care.

Key Responsibilities

As a Psychiatric Liaison Nurse, you will be responsible for:

  • Conducting comprehensive mental health assessments and risk evaluations
  • Developing and implementing individualized care plans to promote recovery and well-being
  • Collaborating with multidisciplinary teams to ensure effective care coordination
  • Providing education and support to patients, families, and carers on mental health conditions and treatment options
  • Participating in quality improvement initiatives to enhance patient outcomes and experiences

Requirements

To be successful in this role, you will need:

  • Registration as a Psychiatric Nurse with the Nursing and Midwifery Council
  • Post-registration experience in mental health nursing
  • Knowledge of mental health laws, policies, and procedures
  • Ability to communicate effectively with patients, families, and healthcare professionals
  • Strong interpersonal and problem-solving skills
