Position Overview

We are pleased to present an exceptional opportunity for a dedicated and innovative Clinical Pharmacy Specialist to become a vital part of our team at The Confederation, Hillingdon CIC on a permanent basis.

The ideal candidate will engage with multiple healthcare practices, closely collaborating with General Practitioners to enhance patient care. This role is designed for a Clinical Pharmacy Specialist who thrives in a team-oriented environment and is eager to contribute to the advancement of pharmaceutical services.

Key Responsibilities

The successful applicant will be an integral member of a multidisciplinary team, responsible for:

  • Conducting comprehensive medication reviews with patients, including those in care facilities and during home visits, to effectively manage polypharmacy and provide recommendations to senior clinical staff.
  • Leading clinics focused on long-term conditions, addressing the medication needs of patients with chronic illnesses such as respiratory, cardiovascular, and diabetes-related issues.
  • Collaborating with care home personnel to enhance the safety and efficacy of medication management.
  • Managing minor ailments and providing guidance on self-limiting conditions while ensuring appropriate referrals to other healthcare professionals when necessary.
  • Offering a dedicated helpline for patients seeking assistance with their medications, ensuring they receive the necessary support.
  • Implementing systems to ensure continuity of medication supply for high-risk patients, particularly those requiring compliance aids.
  • Developing and executing a repeat prescribing policy, overseeing the authorization process for repeat prescriptions.
  • Engaging in quality improvement initiatives through clinical audits and feedback mechanisms.

Qualifications and Skills

Candidates should possess the following qualifications:

  • A completed undergraduate degree in pharmacy and current registration with the General Pharmaceutical Council.
  • A minimum of two years of experience in a pharmacy role.
  • Familiarity with common acute and chronic conditions encountered in primary care settings.
  • Progress towards or possession of an independent prescribing qualification is advantageous but not mandatory.
  • Strong adherence to legal, ethical, and professional standards in pharmacy practice.
