We are seeking a dedicated Personal Advisor to join our team, focusing on assisting young individuals aged 16-25 as they navigate the transition from care to independent living. In this vital role, you will provide essential guidance and support, ensuring that these young people have access to the resources and assistance necessary for a successful future.
Key Responsibilities:
- Deliver personalized advice, empowering young individuals to make educated choices regarding their futures.
- Assess individual needs, coordinate evaluations, and create customized pathway plans.
- Provide guidance on health, housing, career options, and financial literacy, ensuring comprehensive support.
- Collaborate with various agencies in housing, education, and health sectors to ensure integrated support.
Required Qualifications:
- Relevant qualifications in social work, youth work, or a similar discipline.
- Experience working with young people in care, along with a solid understanding of the Leaving Care Act.
- Excellent communication and teamwork abilities.
- Willingness to travel within the designated area.
