Active Care Group, a leading provider of complex care services, is seeking a dedicated and experienced Social Worker to join our esteemed team at the Blackheath Brain Injury Rehabilitation Unit in London.

This highly specialized unit offers comprehensive neurorehabilitation services to patients with physical and cognitive impairments resulting from acquired or traumatic brain injuries. Our multidisciplinary team comprises neurorehabilitation medicine specialists, neuropsychiatrists, neuropsychologists, nurses, physiotherapists, speech and language therapists, occupational therapists, music therapists, and social activities coordinators.

As a key member of this collaborative environment, you will play a vital role in supporting patients to achieve their individual goals and maximize their potential for recovery.

Your Responsibilities:
  • Provide comprehensive case management services to a defined patient population.
  • Ensure adherence to legal frameworks such as DoLS, Court of Protection, Safeguarding, and MCA.
  • Lead the assessment process, develop individualized care plans, and implement interventions to facilitate discharge planning.
  • Collaborate with patients, families, and other healthcare professionals to deliver person-centered care.
  • Maintain accurate and up-to-date health and social care records in compliance with organizational policies.
  • Promote a safe and supportive environment that upholds the rights and well-being of all individuals under your care.
Your Qualifications:
  • Professional qualification (Degree or equivalent) in Social Work.
  • Current registration with the relevant professional body.
  • Demonstrated experience working within legal frameworks related to DoLS, Court of Protection, Safeguarding, and MCA.
  • Successful completion of an Enhanced DBS check will be required.
