We are seeking a highly motivated and skilled Mental Health Practitioner to join our Adult Community Team in South Norfolk and surrounding areas. As a key member of our team, you will be responsible for providing assessment, planning, review, and delivery of care and treatment to service users on your caseload.

Main Responsibilities

You will be expected to work closely with service users, families, and carers to provide a holistic approach to care. This will involve assessing and managing risk, developing and implementing care plans, and providing support and guidance to service users and their families.

Requirements

To be successful in this role, you will need to have a nursing degree or equivalent, and ongoing professional development working towards degree level. You will also need to have experience in mental health practice, risk assessment, and risk management.
