**Job overview**:
We are pleased to offer this opportunity to join the Adult Community Mental Health Team covering South Norfolk and surrounding area.
We provide assessment and treatment for those experiencing an acute mental health need in the community alongside other agencies to ensure the best outcome for our service users. We work with service users with a variety of mental health concerns from the age of 25-75 years of age.
We work closely with families and carers to provide a holistic approach and work in a creative and flexible way to provide a positive outcome for our service users within their recovery journey.

**Main duties of the job**:
As an Assistant Practitioner within the South Adult CMHT you will be working in a supportive team to deliver excellent care.
You will be responsible for the planning, review and delivery of care and treatment in conjunction with the service users on your caseload.
You will support senior staff to provide duty cover for the team on a rostered basis and will support individuals in need and will liaise with other agencies to ensure care needs are met.
You will require excellent communication and organisational skills and the ability to work flexibly in a busy environment.

A Care certificate or completed within first 12 weeks of start date, or relevant equivalent qualification.

Foundation Degree in Health and Social Care / Mental Health at Level 5 or Apprenticeship in Health & Social Care / Mental Health Level 5.
