About the Role

Suffolk Primary Care is seeking a skilled and compassionate Advanced Nurse Practitioner to join our dynamic multi-disciplinary team at Haven Health, on a part-time basis. Located in a coastal town, this role offers a genuine opportunity to make a real difference in improving the quality and safety of care provided to patients.

Main Responsibilities

  • Accurately examine and assess patients with undifferentiated diagnoses, from a physiological, psychological, and social perspective, planning clinical care accordingly.
  • Diagnose and manage both acute and chronic conditions, integrating drug and non-drug-based treatment methods into a management plan.
  • Seek advice and support from senior clinicians as appropriate to inform decision-making around patient care.
  • Prioritize health problems and intervene appropriately to assist patients in complex, urgent, or emergency situations, including initiation of effective emergency care.
  • Review medication for therapeutic effectiveness appropriate to patient needs and administer medication in accordance with PGD/Independent Nurse Prescribing.
  • Provide information and advice on prescribed or non-prescription medication, on medication regimens, side effects, and interactions.
  • Work with patients to support compliance with and adherence to prescribed treatments.
