Key Responsibilities:

This community-focused team offers short-term assistance to individuals in their homes who require urgent support to maintain their well-being. Patients may have recently transitioned from a hospital stay or been referred to our team for timely interventions to avert hospital admissions. The specialized knowledge of a pharmacy technician will significantly enhance the skill set of our current UCR team, which includes occupational therapists, physiotherapists, nurses, community matrons, support workers, therapy assistants, administrators, and in some cases, GPs and paramedics.

This role is dynamic and rewarding, involving patient interaction and responsibilities such as providing patient education, supporting medication adherence, and advising clinical interventions to GP and PCN pharmacy colleagues to enhance patient care. You will deliver a medicines reconciliation service to new patients referred to the UCR teams and assist with the transcription of MAR charts when medication administration is required.

You will be integrated into the broader network of clinical pharmacy teams within the Community Services division and will receive professional support from a community services pharmacist.

Strong communication skills are essential, as is a passion for resolving medication-related inquiries. You will be dedicated to empowering patients to optimize their medication use, providing safe and timely adherence advice, and ensuring effective communication with relevant parties. Flexibility, organization, and the ability to adapt to the evolving needs of patients while prioritizing workload are crucial.

You will engage in mutual mentoring, training, and support with colleagues in the UCR teams and the wider pharmacy team. This position offers excellent opportunities for personal growth for GPhC registered medicines management technicians from any sector.
