About the Role

We are seeking a highly skilled Medicines Management Technician to join our Urgent Community Response Team based at Crediton Hospital. As a key member of our team, you will play a vital role in providing safe and timely adherence advice to patients at home, ensuring onward communication to relevant parties, and providing a medicines reconciliation service to new patients referred to the UCR teams.

Key Responsibilities
  • Provide patient education and support adherence to medication regimens
  • Recommend clinical interventions to GP and PCN pharmacy colleagues to improve patient care
  • Conduct medicines reconciliation services for new patients referred to the UCR teams
  • Assist with the transcription of MAR charts when the team is required to administer medication to patients
  • Work collaboratively with multi-professional colleagues to bring a joined-up Pharmacy service to patients in the Community
