Schoen Clinic Newbridge is seeking a highly skilled and experienced Social Worker to join their team. As a Social Worker, you will play a vital role in supporting patients with mental health difficulties, working collaboratively with a diverse team to provide expert social work input and guidance.
Key Responsibilities- Provide expertise and guidance to the multidisciplinary team (MDT) on social care interventions, taking into account the health and social context of patients.
- Liaise with patients, their families, and other disciplines to monitor, evaluate, and modify treatment plans to ensure effectiveness and progress.
- Monitor patients' ongoing mental state and parenting capability, recognizing any safeguarding concerns and prioritizing the wellbeing of children.
- Develop strong links with agencies to promote social inclusion for patients and carers, and act as a resource for the MDT on identifying and accessing relevant services and unmet needs.
- Promote awareness of the role of social workers within the team, negotiating priorities as needed.
- Work within NICE guidelines, Schoen Clinic policies and procedures, Professional Codes of Conduct, and Health and Safety at Work Act.
